HOW TO SUBMIT
We need current valued loss runs and current motor vehicle reports on all driver with the application. The program requires registration with Azuga for Driver Habits Telematic Program. The cost is $100.99 per truck for each device and $15 monthly charge per truck. You can submit an application to transportation@siuins.com or log in to complete the online form.
